Will the publishing company format your book manuscript for you?

Can publishers add the headers and footer on the right pages, make the margins the right size, change the font size, and change the font type if necessary, all for free? Or do you have to pay them? If so, how much?

Also, will they add in acknowledgement/dedication pages to your work if you want them to?Will the publishing company format your book manuscript for you?
All publishers will format (typeset) your book for you. If you're dealing with a traditional (trade) book publisher, you don't have a choice--they format it how they want it. But if you're dealing with a modern supportive self-publishing house, such as the Polished Publishing Group (PPG), you can either use their design services or design the book yourself. A publisher which accepts a book from an author to offer for profit will do all of those things, and absorb the cost as overhead in anticipation of turning a profit on the book. The same applies to Self-Publishing - the author is the publisher, and absorbs the printing costs .

A vanity press will charge you through the nose for all of these things, because they don't expect the book to sell a single copy, and all their profit comes from the author.
